    A Nexus Letter is a formal clinical document that explains the professional connection (“nexus”) between an individual’s mental health condition and a specific event, experience, or circumstance. These letters are often used in legal, administrative, or benefits-related contexts and must be written with clinical precision, ethical care, and clear reasoning.

    At Leading Light, Nexus Letters are prepared by licensed clinicians and are grounded in clinical evaluation, documentation review, and professional judgment-not advocacy or opinion alone.

    Purpose of a Nexus Letter

    The purpose of a Nexus Letter is to address the question of clinical relatedness, such as whether a mental health condition is more likely than not connected to a particular experience or set of circumstances.

    These letters may be requested in matters involving:

    • Legal proceedings
    • Workplace or institutional claims
    • Disability or accommodation requests
    • Immigration or asylum cases
    • Military, veteran, or service-related claims
    • Civil or administrative reviews

    A Nexus Letter does not determine legal outcomes. It provides a clinical perspective that decision-makers may consider alongside other evidence.

    What a Nexus Letter Includes

    Each Nexus Letter prepared at Leading Light is individualized and may include:

    • Relevant clinical background and presenting concerns
    • Summary of evaluation findings and clinical impressions
    • Review of collateral information, when applicable
    • Discussion of the relationship between symptoms and the identified event(s)
    • Clinical reasoning supported by accepted mental health frameworks
    • Professional opinion stated within appropriate clinical and ethical limits

    Language is objective, neutral, and carefully constructed to avoid speculation or legal conclusions outside the clinician’s scope of practice.

    What a Nexus Letter Is Not

    To maintain clinical integrity, it is important to understand what a Nexus Letter does not do:

    • It is not a guarantee of legal, financial, or administrative approval
    • It is not written solely based on client request or preference
    • It does not replace testimony, forensic evaluation, or legal argument
    • It is not advocacy disguised as clinical opinion

    Leading Light does not provide pre-determined or outcome-promised letters.

    Evaluation and Clinical Standards

    Nexus Letters at Leading Light require an appropriate clinical process, which may include:

    • A comprehensive intake or evaluation
    • Review of relevant records (when available)
    • Clinical screening and assessment
    • Determination of clinical appropriateness

    Not all requests result in a Nexus Letter. If the available information does not support a clinically defensible opinion, this will be communicated transparently.

    All Nexus Letters are prepared in accordance with:

    • Professional ethical standards
    • Scope of practice requirements
    • Documentation and record-keeping guidelines
    • Clinical neutrality and objectivity

    Who May Benefit from a Nexus Letter

    • Have been advised by an attorney, advocate, or agency to obtain one

    All requests are screened for appropriateness.

    A Nexus Letter reflects a clinician’s professional opinion based on available information at the time of evaluation. It does not predict outcomes, override legal standards, or substitute for judicial or administrative decision-making.

    Getting Started

    Individuals seeking a Nexus Letter must complete an intake and evaluation process prior to consideration. Additional documentation or records may be requested to support clinical review.

    Pricing & Process

    Nexus Letter Fees

    Nexus Letters require clinical time beyond standard therapy services and are therefore not billable to insurance. All Nexus Letters at Leading Light are offered on a self-pay basis.

    Current Fees:

    • Nexus Letter Evaluation & Clinical Review: $350
    • Nexus Letter Preparation (written report): $450
    • Total Standard Fee: $800

    Additional fees may apply for:

    • Extensive record review (medical, legal, employment, or military records)
    • Supplemental letters, revisions requested after completion, or addendums
    • Expedited turnaround requests

    All fees are discussed in advance and must be paid prior to the release of any written document.

    Nexus Letter Process

    To maintain clinical and ethical standards, all Nexus Letters follow a structured process:

    1. Initial Inquiry & Screening
      Requests are reviewed to determine whether a Nexus Letter is clinically appropriate.
    2. Intake & Evaluation
      The individual completes a clinical intake and participates in an evaluation with a licensed clinician.
    3. Record Review (if applicable)
      Relevant records may be requested to support clinical review and opinion.
    4. Clinical Determination
      The clinician determines whether a Nexus Letter can be ethically and professionally supported.
    5. Preparation of Nexus Letter
      If appropriate, the letter is written using objective, clinically sound language.
    6. Delivery
      The completed Nexus Letter is provided securely upon finalization.

    Completion of an evaluation does not guarantee that a Nexus Letter will be issued.

    Requests that do not meet clinical, ethical, or professional criteria will be declined.

    Insurance & Reimbursement Notice

    Nexus Letters and related evaluations are not covered by insurance and are not considered medically necessary treatment services. Clients may not submit these services for reimbursement unless advised otherwise by a third party.
